What is terminal care
The terminal phase is a part of palliative care. This is the period in which the body continues to deteriorate noticeably
deteriorate and it becomes clear that time is limited. In this phase, care shifts increasingly towards
comfort, rest and relieving symptoms. Support can become more intensive when needed.
Personal care coordination
You will have a dedicated nurse who will keep an overview of all the healthcare providers involved.
This nurse monitors changes in your health and coordinates with doctors.
This keeps care clearly organised for you and your family.
Advanced symptom monitoring
Complaints such as pain, tightness, nausea or fatigue may change.
Our nurses observe these changes and, when necessary
with the attending physician so that symptoms are relieved as much as possible.
Subcutaneous medication and nursing procedures
Some medications work best when administered under the skin.
Our nurses can provide injections and support wound care,
catheter care and other nursing actions so that care at home remains safe.
Recognising changes
Small changes can mean a lot. Our nurses watch for signs
such as fatigue, changes in breathing or pain.
By recognising these signals early, support can be adjusted faster.
Counselling for restlessness or anxiety
At this stage, feelings of restlessness or confusion may arise.
Our nurses provide calm guidance and help situations
become more manageable with attention to emotional well-being.
Comfort-oriented daily care
Besides medical care, daily life remains important.
We support personal care, eating and drinking
and provide a comfortable daily routine to suit your situation.
Advanced care planning
Together, we discuss wishes around care and treatment.
Looking ahead provides clarity and peace of mind
for both you and your loved ones.
Night and wake care
Extra presence at night can give peace of mind.
A caregiver can support with symptoms,
care or simply be present when it offers reassurance.
Family involvement
When relatives do not live nearby, distance can cause concerns.
With your consent, we will keep loved ones informed by phone or video call,
so they remain involved in your situation.
Practical completion and aftercare
When a period of care is completed, we remain available
for practical questions. A wrap-up conversation can help
to look back on the care period together.
In short
Palliative home care is there to relieve symptoms and bring peace of mind
in a vulnerable period. With professional nursing care,
clear coordination and personal attention, we provide
safe support at home.
